I declare that I am medically fit to drive and have no condition or taking any prescribed medication that will compromise my driving licence obligations

I have a full valid licence (with no disqualifications) for the type of vehicle I am being asked to drive

I will not drive a vehicle whilst under the influence of drink or drugs (whether illegal or prescribed)

I will comply with any health and safety requirements on this site and any site visited during my shift

I will ensure that the load is secure and is within the maximum permitted weight of the vehicle

I understand and will comply with speed limits of roads and vehicles and all other road traffic legislation

I understand and will follow the requirements around vehicle checks (pre and post use) and the sites defect reporting process

I will return the vehicle and its equipment in the same condition as I found it in

I will ensure my digital drivers card is downloaded at the required levels and at the end of my shift, before leaving site

I will ensure that any legally required printouts or written manual records, which relate to my placement, are returned to the Menu Partners within the required time
